Aden Perry Good Samaritan and Scholarship Fund - 2025 Scholarship Application

This session will be for the June SAT Test. All classes will be on-line via zoom 5:00-7:00 PM

In a paragraph form, please answer: "What is the significance of this scholarship opportunity, and what is the

importance of getting as high of a score possible on your SAT test. (What impact will it have on your future)?"

SUBMIT YOUR NAME, GRADE, SCHOOL AND EMAIL WITH YOUR APPLICATION

Rules:

• You must take the complete course.... all 20 hours of the classes (10 days, 2 hours each ONLINE VIA ZOOM)

• You must have your computer screen ON AT ALL TIMES so that a live image of your face is always visible....NO

BLACKED-OUT SCREENS OR STILL IMAGES WILL BE ALLOWED

• You must be on time for class. After the check-in / attendance process is completed, no one will be granted

access to the class. It is requested that, if possible, you be in the waiting room ready to be let in 10 minutes

before class begins

• You must log-in to class with the same name that you have applied for the class with. Attendance will be taken

off the list submitted with the names as registered.

• The email address you register with must be your own personal email as that will be where all correspondence,

class updates and log-in information will be sent

• You and your parents must sign this form and submit it with your paragraph by FRIDAY, APRIL 25TH

• You must arrange to get your book from me or from your BRACE Advisor PRIOR to the 1st class on MAY 3RD.

• YOU MUST REGISTER ON-LINE FOR THE JUNE TEST PRIOR TO THE START OF THE FIRST DAY OF CLASS

• You must pay for your testing fee or obtain a voucher from your BRACE advisor PRIOR to the 1st class

• ***FEES WILL BE REIMBURSED AFTER YOU TAKE THE CLASS***

• PLEASE SAVE YOUR TEST FEE RECEIPT IF CHARGED SO THAT AFTER YOU COMPLETE THE CLASS AND TAKE THE

TEST YOU CAN SUBMIT IT VIA EMAIL WITH YOUR ADDRESS FOR AN IMMEDIATE REFUND FROM THE

FOUNDATION
